YogaTribe Consent Form

Acknowledgment of Risks:

​

I acknowledge that participating in yoga classes or workshops involves physical activity and carries inherent risks, including the risk of injury but not limited to physical injuries, property damage, and the effects of any strenuous activity or exertion.

Voluntary Participation and Personal Responsibility:

​

I voluntarily choose to participate in yoga classes or workshops. I understand that it is my responsibility to inform the instructor of any physical limitations or health concerns that may affect my participation. I acknowledge that I should not overstretch or exert myself to the point of pain, but it is my sole responsibility to practice safely. I understand that the instructor or Yogatribe will not be held responsible for any injuries.

Release of Liability:

I hereby release, discharge, and hold harmless to YogaTribe Holistic Services PTY LTD, its owners, instructors, and staff from any liability, claims, demands, actions, or causes of action arising out of any injury, loss, or damage that may occur as a result of my participation in yoga classes or workshops.

Confidentiality:

​

I understand and agree to respect the privacy and confidentiality of other participants. I will not disclose any personal information shared by fellow participants during the classes or workshops.

​

Photography and Recording:

​

I consent to the use of photography, video, or audio recordings during yoga classes or workshops for promotional purposes on facebook, Instra and other media by YogaTribe Holistic services pty ltd. I understand that if I do not wish to be photographed or recorded, it is my responsibility to inform the instructor in advance.

Doctor Clearance and Health Conditions:

​

I understand that it is my responsibility to obtain clearance from a healthcare professional before starting yoga practice, particularly if I have any pre-existing health conditions or concerns. I acknowledge that I must inform the instructor about any ailments or health issues I may have through the provided questionnaire or by directly discussing with the instructor.

Online Yoga Practice and Video Screen:

​

I agree to keep my video screen on and clearly visible during online yoga sessions. I understand that this is necessary to receive guidance from the instructor and to avoid any potential injuries. I acknowledge that the instructor or Yogatribe will not be held responsible for any injuries that occur if instructions are not followed properly.

​

Fees, Refunds, and Makeup Classes:

​

I understand that the fees paid for yoga classes are non-refundable, non-transferable, and credits cannot be carried forward when enrolled through offer price. Full payment is required for this offer, no spilt payments option.

​

Make-Up Sessions

​

No makeup sessions as its unlimited classes offered for a year.

​

Membership Freeze

​

No option to freeze the membership once signed up. I acknowledge that if I am unable to attend a class offered, a replacement class will not be allocated.

​

Non-Usage of Classes:

​

I understand that non-usage of any class, whether due to cancellation, late arrival, or no-show, will be counted as a leave/absence. I acknowledge that I will not be entitled to any refund or compensation for missed classes.

​

Session Timings and Rescheduling:

​

I understand that every effort will be made to adhere to the session timings. However, in the event that a regular teacher is unavailable, classes may be rescheduled or taken by an available instructor. I acknowledge that the yoga center reserves the right to make such changes, and I will be notified accordingly. I acknowledge that no refund will be issued if I decide to discontinue in such cases.

Parking

​

I understand that we should park in Visitors car slot only and not in any other parking slot. The Cars will be towed at owners’ expense if it is parked in others unit slot.

Rights of Admission:

​

I understand that Yogatribe reserves the right to cancel the admission of participants for any abusive behaviour towards staff or other members. I acknowledge that such behaviour may result in the cancellation of my admission.
